IN HOME CASE MANAGER
Description of Agency:
Family Services of Davidson County is a non-profit agency located in Lexington, NC providing counseling, crisis intervention, and youth development programs to strengthen individuals, families, and our community. We work from a team model and Trauma-Informed Care approach to provide optimal services to our clients and to promote a healthy workplace environment.
Primary Function:
The In-Home Case Manager will provide support, advocacy and case management services to crisis intervention clients experiencing domestic or sexual violence. They will also assist with filing for a Domestic Violence Protective Order and advocacy to victims of domestic violence and sexual assault and support volunteer advocates. The In-Home Case Manager works under the direct supervision of the Victim Advocacy Coordinator.
Education/Experience:
Requires knowledge and belief in “Housing First” philosophy and strategies.
Must have direct service experience with individuals who have been victims of domestic violence and/or sexual assault.
Working knowledge regarding the causes, consequences, and dynamics of domestic violence, sexual assault, and child abuse and neglect.
A Bachelor’s degree from an accredited school of social work or other human services related field is preferred.
Accountability:
Works under the direct supervision of the Victim Advocacy Coordinator
Responsibilities:
Assists in developing a strength-based/solution focused goal and action plan that promotes safe housing and self-sufficiency.
Conducts ongoing evaluations of the client’s progress through the service plan, reassessment and adjustments to the plan.
Provides vital in-home case management for Safe at Home clients when requested by clients. In some instances, the client may request case management on-site, however, if a client is unable to transport themselves to the office, it is mandatory that in-home case management is provided.
Provides appropriate services in answering the 24-hour domestic/sexual violence crisis hotline.
Provide shelter coverage as needed.
Maintains a flexible schedule to include on-site emergency appointment times and consults as needed on a weekly basis.
Maintain a role to empower our clients and to assist them to meet their housing and safety planning goals.
Will join the shelter case manager when meeting with potential Safe at Home clients to build a rapport with the clients entering the program.
Provides assistance to clients completing protective orders and provides advocacy services in civil and criminal district courts.
Will be on site at the Thomasville location to provide advocacy, case management, accompaniment, and support to the volunteer advocates.
Provides assistance in filing for Victim’s Compensation.
Maintains documentation for all contacts made with FSDC clients.
Attends monthly crisis intervention team staff meetings.
Coordinates with the crisis intervention team to ensure timely and appropriate treatment and support for all clients.
Provides educational presentations and consultation to agencies and organizations that serve family violence victims when requested by Victim Advocacy Coordinator, Shelter Coordinator or Deputy Director.
Assists in agency fundraising activities and community awareness events.
Serves on agency and community committees as needed.
General:
· Promote the Family Services philosophy to the community.
· Process all correspondence, time and travel sheets, and monthly reports as scheduled.
· Represent agency at meetings when requested and participate in fundraising events.
· Carry out any additional assignments as requested by the supervisor or director.
COVID-19 considerations:
Staff are required to show proof of full COVID-19 vaccination within 90 days of employment unless they qualify for a religious or medical exemption. Clients are screened through a questionnaire. Staff are provided with PPE and cleaning supplies. Clients have options for in-person or telehealth appointments.

CONTRACT THERAPIST
Description of Agency:

We work from a team model and Trauma-Informed Care approach to provide optimal services to our clients and to promote a healthy workplace environment.
Primary Function:
Provides comprehensive mental health assessment and ongoing counseling for individuals, families, and groups. Services are both in-office and community-based. Therapist will also serve as an advocate for clients by coordinating care with primary care physicians, school personnel, Department of Social Services social workers, and other professionals/agencies involved with the clients and their families.
Education/Experience:
· A master’s degree from an accredited school of social work or counseling program. Independent licensure in the state of North Carolina (e.g. LCMHC, LMFT, LCSW)
· Demonstrated competence as a clinician who has solid theoretical base in the helping process that includes the Family Systems approach and understanding of Trauma-informed care.
· Demonstrated ability to function as a consultant, educator, and advocate.
· Demonstrated ability to form a therapeutic alliance with clients and to possess a strong commitment to human services as a profession.
Accountability:
Works under the direct supervision of the Lead Therapist
Responsibilities:
Maintain 15-20 direct service hours in individual, family, and group therapy services to various populations as appropriate within the criteria for clinical services. Can see more than 20 direct service hours if would like.
Maintain accurate and current records on all clients as required by the agency. This includes writing appropriate treatment plans, social histories, assessments, and electronic progress notes.
Complete in a timely fashion all necessary reports regarding services provided, time sheets and any other information requested by the agency management.
Maintain a flexible schedule to include an emergency appointment time and consults as the need dictates on a weekly basis.
When necessary, provide advocacy services in school-based care meetings and, if necessary, in civil and criminal district court.
Continue professional growth through attendance at supervision, workshops, and staff meetings. This includes the use and continued growth of the professional self.
Maintain professional credentialing or licensure requirements of the specific therapist's professional field and follow their ethical guidelines.
Co-ordinate services with other community resources for our clients and advocate for them when needed.
Provide consultations to agencies and organizations that serve families or children.
General:
Promote the Family Services philosophy to the community.

Represent agency at meetings when requested and participate in fundraising events.
